Middle School Sports Schedules

LMS Sports Clearance Requirements:

Please submit all paperwork to the nurse's office PRIOR to the start of tryout/practice dates in order to be eligible to participate in any Winter, 2025 - 2026 sports offering.  Lawrence Township District physician will clear all physical paperwork beginning in February. 

Spring 2026 Tryout/Practice Dates:

Softball Tryouts: March 23, 24 and 25.  First Practice - April 6th. 

Baseball Tryouts: March 23, 24 and 25.  First Practice - April 6th. 

Spring Track and Field: First Practice - April 7th. 

All student athletes MUST be cleared prior to tryout dates/participation. 

Sports Clearance Steps:

1. Complete Online Registration - Click HERE to access.

2. Submit only the medical eligibility form - page 1.  All other physical paperwork should be kept on file at your child's doctor's office - if the most recent physical is older than 365 days old.  Click  HERE to access paperwork. 

3.  Health Update Form - If the most recent physical is older than 90 days  -  Click HERE to access paperwork. 

4.  All LMS student athletes are required to complete the online baseline assessment in order to participate in athletics.

5. Submit all paperwork to LMS Nurse's office.
