Have you gotten your athletic physical yet? Don't wait until it is too late!
Students must have a physical on file in the Nurse's Office in order to participate in any sport. The physical must have been completed within 365 days of the first day of practice. NOTE: If your athlete has had a physical completed by your private physician, the school physician must still clear them for participation in sports. (N.J.A.C. 6A;16-2.2)

LMS: Call Cynthia Reagan R.N, CSN at 609-671-5523 or email creagan@ltps.org.***All physicals for the 2024 Spring season completed by YOUR private physician must be turned into the nurse's office by Monday, March 4, 2024 to be cleared for the start of Baseball, Softball, Golf, Lacrosse, T&F and Tennis practices.
If you miss the 3/4 deadline, you can submit your physical paperwork on 3/11 for approval on 3/12 or on 3/18 for approval on 3/19. Keep in mind, the spring season begins on 3/11 - Baseball/Softball pitchers and catchers and Tennis - 3/14 for all other Baseball/Softball positions and every other spring sport. No athlete may participate in any athletic offering without being both medically and academically cleared first.
ONLINE REGISTRATION FOR ALL SPORTS
Anyone interested in participating in a sport must also have the Online Registration form filled out prior to the start of the first practice. If you do not have online access or need help filling out the form, please see your school nurse or come to the Athletic Department.

STEP 2:
- If you HAVE NOT had a physical within the last 365 days, please download, complete and return the physical form to the Nurse's Office prior to the deadline date. Form can be found on https://lawrence-ar.rschooltoday.com/
- If you HAVE had a private physical within the last 365 days and it is on file in the Nurse's Office, please download, complete and return the Health History Update form to the Nurse's Office prior to the deadline date. Form can be found on https://lawrence-ar.rschooltoday.com/
Please make sure your doctor has completed the Student-Athlete Cardiac Assessment Professional Development Module, a requirement for any doctor completing a physical for a LTPS student. Scholastic Student-Athlete Safety Act Information
Prior to participation on a school-sponsored interscholastic or intramural athletic team or squad, each student-athlete in grades 6-12 must present a completed Preparticipation Physical Evaluation (PPE) form to the designated school staff member. Important information regarding the PPE is provided in the file below, and you should feel free to share with your child’s medical home health care provider.
